Pathway Planning Support: Life Transitions
Pathway Planning is a structured, therapeutic support designed to help neurodivergent young people prepare for and navigate key life transitions. Moving from adolescence into adulthood brings evolving expectations, increasing responsibilities, and exposure to complex systems such as education, employment, housing, and health. For many young people—and their families—this can feel overwhelming without the right guidance.
Our Pathway Planning Support provides the clarity, confidence, and structure needed to make this transition a meaningful and empowering experience. Grounded in trauma-informed and neurodiversity-affirming practice, this service offers more than just future planning—it builds the foundational skills and emotional readiness to support long-term wellbeing and independence.
Why Pathway Planning Matters:
Without tailored support, many young people risk falling through the cracks during transitional stages. These critical periods can influence a person’s lifelong relationship with work, identity, relationships, and self-determination. A proactive, person-centred approach ensures young people feel supported—not rushed—as they shape their own path forward.
What We Offer:
Each pathway program is co-designed with the young person and their support network to reflect their unique strengths, needs, and aspirations. Areas of focus may include:
- Exploring identity, interests, values, and future goals
- Understanding options for education, training, or employment pathways
- Developing independent living skills (e.g., routines, budgeting, decision-making)
- Preparing for transitions such as finishing school, moving out, or accessing adult services
- Building self-advocacy, communication, and emotional regulation skills
- Connecting with local community supports, peer networks, and services
Rather than delivering a generic plan, we work collaboratively to create a flexible, strengths-based roadmap that evolves alongside the young person. Whether they’re just beginning to think about the future or ready to take the next step, our goal is to ensure they feel capable, connected, and in control of their journey.
